Sub Acute Services

In addition to therapy, we are the only facility in the area with a sub-acute unit for residents who require a higher level of care than skilled nursing.  This is a unique service that we are able to offer because of the clinical expertise of our staff.  Our Registered Nurses work closely with our physicians to determine and execute the best plan of care for each individual sub-acute patient.  Services include:

Nursing Services

At Legacy Post-Acute, we have assembled a team of the most compassionate and skilled nurses in the valley. They truly live by our mission statement to be “Committed to 100+1 Service” consistently going the extra mile to ensure our patients are treated with love and respect, and that their clinical needs are met.

Our nursing team consists of RN’s, LVN’s, and CNA’s. We have RN’s on duty around the clock to ensure the most experienced clinicians are attending to our patients’ needs. Our nurses work in close coordination with our physicians to discuss and address the clinical needs of the patients. They receive weekly training on the latest nursing protocols, methodologies, and medical advancements and are continually trained on clinical procedures and patient care.

Rehabilitation Services

We have the best therapists in the San Bernardino Valley. Our therapists are experienced, driven, and excellent at motivating our residents to work hard and restore the strength they need to fully function in their daily lives.

Physical Therapy

Our licensed Physical Therapists focus on improving the coordination, balance, strength, endurance and joint flexibility to achieve optimal levels of functional mobility, either unassisted or with the aid of an assistive device.  Our staff is fully trained on Wound Care Management using ultrasound, diathermy, sharp/mechanical debridement and electrical stimulation.  Our Physical Therapists design individualized treatment programs for alleviating pain, preventing disability and restoring function following injury or illness that may include:

  • Joint Mobilization

  • Orthopedic Rehabilitation
  • Wound Care Management
  • Gait and Transfer Training
  • Prosthetic Training

Occupational Therapy

The focus of our Registered Occupational Therapists is to enable the patient to adapt their physical and cognitive capabilities to the skills needed in activities of daily living. Occupational Therapists assess the areas needed for functional independence, customizing a treatment plan with emphasis on balance, strength, perception, and cognition.  Therapy may include:

  • Hand Rehabilitation
  • Activities of Daily Living
  • Cognitive Perceptual Retraining
  • Transfer and Mobility Training
  • Energy Conservation Techniques
  • Utilization of Adaptive Equipment

Speech Therapy

Our licensed Speech-Language Pathologists focus on restoring the patient’s ability to communicate. They address communication problems by helping patients improve skills in speaking, listening, thinking, reading, writing, problem solving, as well as organizing and sequencing thoughts.  Speech-Language pathologists also assess swallowing difficulties and develop treatment and management programs.  This condition, if left untreated, often leads to aspiration of food into lungs, pneumonia and other respiratory problems.  Therapy focuses on:

  • Hearing Screening
  • Reading and Writing
  • Swallowing Retraining

Activities and Social Services

A positive and uplifting atmosphere is vital to the success of healing, recovery and rehabilitation. Legacy Post-Acute Rehabilitation Center employs outgoing and friendly professional staff to create social programs and activities to fit the resident needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Friends and families are always welcome to visit and attend our special events.

Social Services

Social services provide essential information, manage requests and concerns, help in care planning and discharge planning for each resident. We can arrange professional services for:

  • Counseling and social services
  • Family support
  • Assistance with billing, Medicare and coinsurance
  • Personal laundry and linen services
  • Newspaper delivery available
  • Small pets welcome to visit
  • Barber and beauty salon services
  • Accommodations for therapeutic diets with enhanced dining
